Getting started with Kingdom Pet Boarding is easy. Whether you’re a new client registering your dog for the first time or an existing client scheduling an appointment, the forms below help us prepare for your visit and provide the best possible care.
Completing the appropriate form in advance allows for a smooth check-in process and ensures your dog’s experience is safe, comfortable, and well organized from the start.
New Client Registration
If this is your dog’s first visit to Kingdom Pet Boarding, please complete the New Registration form. This form allows us to gather important information about your dog, including health history, vaccination records, feeding instructions, and emergency contacts.
Registering in advance helps our team get to know your dog before their stay and ensures we’re fully prepared to meet their individual needs.
Schedule an Appointment
If your dog is already registered with us, use the Schedule Appointment form to request dog boarding or dog daycare services. This form allows you to select preferred dates, services, and any special instructions related to your dog’s stay.
Appointments are required so we can maintain appropriate staffing levels and provide attentive, personalized care for every guest.
